Children’s Long-term Waiver

Providing the best possible care for a child with disabilities can be a stressful and daunting challenge for families. That’s where the Children’s Long Term Support Waiver Program comes in to help fill the gaps. Often, the child requires additional services or equipment that enables the family to provide the best possible care in their home or community. Our goal is to equip the child and family with the appropriate support and services so that the child may remain in the home and be an integral part of our community.

  • Contact the eligibility specialist at (262)-605-6599. The consultant will:
    • Explain the children’s programs available.
    • Discuss the benefits of the program.
    • Outline the eligibility process.
    • Send an application packet if you wish to apply.
  • Complete the application packet or ask the consultant for assistance.
  • Call the eligibility specialist to set up a home visit after completing the paperwork. The home visit includes:
    • Reviewing the application materials.
    • Gathering additional information.
    • Verifying certain documentation.
    • Answering questions.
    • Giving the consultant an opportunity to meet the family and child.
  • The specialist will determine eligibility and..
    • Help connect you to community resources while waiting for the eligibility determination.
    • Inform of any changes in contact information
    • Send you a letter with all the programs you are eligible to receive.
  • If eligible, you will be contacted by our staff to continue the application process which includes:
    • Home visit to complete a comprehensive assessment of your child’s needs.
    • Determine how the program can assist with meeting your child’s and family needs.
    • Create a service plan.
